1. Approve the minutes from the Board of Supervisors’ meetings held on December
6, 2016 and December 13, 2016.

2. Ratify and/or approve all warrants, electronic fund transfers, and other payments

6. Approve transfer of $851.38 from the Career Center Workforce Activities Fund
1881 and the Workforce Innovation and Opportunity Act Fund 1879 and the

7. Set for public hearing to be held at the Board's meeting of January 24, 2017 at
6:00PM Case No. ZC-16-001: A request for a Zone Change from CG-10,000
(Commercial General) to CN-2/A (Neighborhood Commercial, 2 acre minimum
parcel size) for 2.21 acres. The property is located at 610 S. State Route 89A and
identified as Assessor's Parcel Number 116-08-003G. Applicant: Viola's Flower

8. Approve the renewal of an Intergovernmental Agreement (IGA) between the
Arizona Department of Housing, Office of Manufactured Housing (OMH) and
Coconino County, delegating authority to the Community Development
Department to permit and perform inspection services for the installation of

9. Approve Resolution 2017-05, approving the Final Plat for Johnson Ranch
subdivision, Phase One, Case No. SUB-15-002, property is located off State
Route 89 at Johnson Ranch Road, Assessor’s Parcel Number is 301-31-003A;

10. Approve Resolution 2017-08, authorizing Coconino County Community Services
to administer the Community Development Block Grant (CDBG) funds for
Contract No. 88C-16 between the City of Flagstaff and Community Services for
housing stabilization to City of Flagstaff residents in the amount of $24,600.00.

11. Approve extension of a contract between Coconino County and the Arizona
Department of Housing from December 1, 2016 to August 1, 2017 to continue rehabilitation services through Community Development Block Grant (CDBG)

12. Approve an authorization to enter into a Sub-grant Award Agreement with the
Arizona Department of Public Safety to provide staffing for the increased
recruitment of CASA Volunteers in the amount $39,872.00, for the period of

13. Approve Resolution 2017-02, accepting an Arizona Department of Education
Grant for 2017 Coconino County “Title III Consortium” in the amount of
$32,078.55, for a service period of October 1, 2016 through October 30, 2017

14. Approve Resolution 2017-01, accepting the Arizona Department of Education
2017 FASTER grant to Coconino County Education Service Agency awarded on
September 1, 2017, in the amount of $20,187.28 and, corresponding budget
adjustment for the purpose of receiving and spending local, state, and federal
monies to provide programs and services to school districts and charter schools

15. Approve a budget adjustment for Coconino County “Small Schools Service
Program” in the amount of $150,674.90 to provide support services to Chevelon
Butte School District, Coconino County Regional Accommodation, Fredonia-
Moccasin Unified, Grand Canyon Unified, Maine Consolidated and Williams
Unified District for a period of October 1, 2016 through October 30, 2017.

16. Approve a Budget Adjustment in the amount of $625,548.94 and IGA for
Coconino County for partnership funding of grant award- “Functions and
Statistical Models” Improving Teacher Quality grant and accept the partnership of
the CCESA with Northern Arizona University (NAU); the grant period is from
December 19, 2016 – June 15, 2018. Superintendent of Schools

18. Approve a FY17 Budget Adjustment in the amount of $2,069 in cost center 1307-
31-3030-000-20-434-35-4315 allowing Coconino County Public Health Services
District (CCPHSD) to fully draw down the $12,000 grant awarded from the
Arizona Department of Health Services (ADHS) to provide services related to the
control of tuberculosis. Health District

19. Approve an Independent Contract Renewal Amendment #2 with Axis Forensic
Toxicology (AXIS), formerly known as American Institute of Toxicology, Inc.
(AIT), to extend the agreement for one year, from July 1, 2016 to June 30, 2017,
in an amount expected to not exceed $100,000. Health District

Approve Amendment 6 to IGA DI16-002119 between Coconino County and
Arizona Department of Economic Security to accept a reallocation of Workforce
Innovation and Opportunity Act (WIOA) funds in the amount of $121,311 to
provide employment, training and education services to individuals and
employers, and the corresponding budget adjustment to Fund 1879. Career
